MPCA provides technical assistance to help health centers achieve Patient Centered Medical Home recognition, primarily through coaching on systems-level quality improvement transformations. MPCA provides readiness assessments, improvement suggestions, and feedback on documentation for NCQA PCMH recognition.
Transforming a health center to become a patient centered medical home involves reorganizing care so that the patient is at the center, and all his or her needs are met by a coordinated team of practioners. These needs include preventive care, treatment for acute illness and chronic conditions, and cultural and family needs. Patients are encouraged to learn about their health and be an active partner in their care. By coordinating a patient’s care, providers improve their patient’s experience and outcomes.
