This session lays the groundwork for understanding Chronic Care Management (CCM) and why it matters in the FQHC setting. Participants will walk through the basics of what CCM is, who qualifies to bill for it, and how to enroll patients. We’ll also explore the foundational elements needed to run a compliant and effective program, including care planning and documentation.
