NCQA is taking public comment on their proposed updates to 2026 PCMH Recognition that would go into effect in 2026. Proposed changes include:

• The addition of 9 new elective criteria to the program, center on virtual care.
• Retiring 9 criteria from the program.
• Aligning with Health Plan Accreditation.
• And most impactful: adding 46 cadence thresholds.
